Other documents make reference to a “formal agreement” with EMRTS.

How often have the staff of Velindre University NHS Trust contacted EMRTS since the service’s launch? 
How often has a FULL EMRTS response (Vehicle / Helicopter, CCP, and Doctor) attended a patient within the Velindre Centre, and dates of attendance, Please? 

Can any policies, documents, procedures, and any other relevant material concerning referring patients to EMRTS be forwarded please.

Can you forward any PRESENT policies, documents, procedures, and any other relevant material concerning the recognition, management, referral and transfer of deteriorating patients please?

Are any NHS Hospital Medical Professionals paid in a private capacity for completing any Hospital Death Certificates, please provide details. 
Are all such private fees/payments made, accepted by the Medical Professional involved. Please can you indicate how many payments are not accepted. 
Where such payments are not accepted by the Medical Professional completing the death certificates, are the costs reduced to the family/estate of the deceased patient.

I would like to submit an FOI request for the amount of human albumin in grams distributed to NHS Wales Hospitals in 2020 by Welsh Blood Service (the full calendar year), broken down by manufacturer (Bio Products Laboratory, Grifols, Biotest, CSL Behring, Octapharma, Takeda) and by concentration (4.5%, 5%, 20-25%).
Since I requested the above information about the amount of albumin distributed to NHS hospitals, could you provide me with the information on the number of vials for each of the presentations mentioned in the document you sent me?
