The purpose of the Keep Me Safe app is to remove the need for patients to wait at home for a phone call from the VAP assessment team if they are fit and well.
The Keep Me Safe app is a checklist titled “SACT/Chemo checklist”. The app will ask you a series of questions about your current health and wellbeing, specifically since your last course of treatment. The VAP team can then use these answers, along with your blood test results, to help assess if you are well enough to attend for your next course of treatment.
The Keep Me Safe app will identify any problems since your last treatment through the answers you provide and will highlight them to the assessment team who will contact you and discuss these problems.
Please note: This service is solely for recording your problems after a SACT/chemotherapy treatment. Should you experience any symptoms following your SACT/chemotherapy treatment please contact the Velindre treatment helpline or if you have any other medical issues, you should contact your GP.
No. Participation in this pilot is optional and for your convenience. There is no charge for you to use the app. If you do wish to be a part of this pilot, you will be asked to complete our registration process, which includes signing the form below and downloading the app onto your phone. If you decide to take part and later change your mind, you are free to withdraw from the Velindre Keep Me Safe app immediately, at any time and without giving a reason. There will be clear guidelines on the form below regarding how you can withdraw.
This pilot is only currently available for use on iPhones or iPads with iOS version 14 or above. If you do not have an iPhone or an iPad, unfortunately this pilot is not currently available to you.
A decision to withdraw or a decision not to use this service will not affect the standard of care you receive at Velindre. You will return to or continue to receive phone calls to assess if you are fit for treatment.
You will need to download the Velindre version of the Keep Me Safe app. This app has been provided by a company called Galactig who have developed it in partnership with our clinical staff to ensure it is informative and safe for patients to use.
Once you have downloaded the Keep Me Safe, you will receive a unique access code from the VAP Team. This will only need to be entered once into the app. You will then receive a notification onto the lock screen of your phone advising you when the checklist is open and ready for your responses. A training video accompanied by user help guides will be available to assist you once you have opted in.
The Velindre Keep Me Safe App will not hold any of your health and personal data, such as name, address, or any medical history you have at Velindre or any other Health Board or General Practice.
If you wish to be part of this pilot, please fill out the opt-in form at bottom of this document and send it with the pre-addressed and stamped envelope.
Once you have confirmed that you wish to take part, a member of the VAP Team will contact you via email and advise of the next steps, including how to download the Velindre instance of the Keep Me Safe app.
You can decide to withdraw from using the Velindre Keep Me Safe app at any time. There is no personal information held on either the Keep Me Safe app or the Dashboard that the VAP Team use. The answers you select from the multiple-choice options provided will be held on the Keep Me Safe Dashboard. You may continue with your regular form of communication and treatment assessment with your healthcare team.
Should you wish to withdraw from the Velindre Keep Me Safe App Pilot, please contact the VAP team and/or contact the Velindre University NHS Trust (VUNHST) Data Protection Officer – as per the contact details provided in section 9.
There is no personal data held within the Velindre Keep Me Safe app. This app also sits outside of the NHS Medical Record systems so there is no direct linkage with your app usage and your personal medical record. Your registration data is pseudonymised (which is a form of encryption) and only specific members of staff at Velindre will be able to read this information.
The answers you provide to the questions asked within the app will be stored and viewed by your clinical team at Velindre within a secure patient dashboard, The information will not be automatically added to your medical record.
There is no access or linkage to your Velindre Medical Record or wider NHS record via this app.
Your participation in the Velindre Keep Me Safe pilot is based on an opt-in model, meaning that Velindre invites you to voluntarily join this pilot. Your decision to participate is optional and you can choose to withdraw from the pilot at any point. The level of care you receive from Velindre remains unaffected regardless of your choice to participate. It is important to note that participation in this pilot is not mandatory for continued care and treatment at Velindre. If you have any inquiries or wish to withdraw from the pilot program facilitated by Velindre, please reach out to the VAP Team, or contact the Velindre University NHS Trust (VUNHST) Data Protection Officer – as per the contact details provided in section 9.
