If you no longer want to participate or wear the wristband, we would kindly ask for you to return it in the prepaid envelope included within this booklet.
If you sadly deteriorate and pass away, we will delete the information on the wristband. But to ensure this is done in a timely manner, it would be helpful for someone to let us know. If your family feel able to return the wristband to us, this allows us to reset and re-use the domain for a future patient.
