This initiative is where suitable patients are invited to wear a QR-code wristband as a way of sharing important medical information to different healthcare professionals. It is completely voluntary and free.
This is a new and innovative way of allowing your important medical information to be readily available to healthcare professionals whenever it is required. By wearing the wristband as if it were a watch, you will be carrying this with you wherever you are, making it available to a healthcare professional who might need it, for example, in an emergency.
Each patient care wristband has a unique QR-code on it (as seen in picture 1) which, if scanned, links to a secure website with preloaded information that is unique to you that can only be accessed by a secure PIN which is kept on the inside of wristband.
If you decide to take part, we will sit down with you to help upload your important medical information to your individual site. Only information you are in agreement with will be uploaded.
