What is an investigation?

All concerns, including patient safety incidents, must be managed and investigated in keeping with the Regulations.  The investigation must review what happened, why it happened and identify any learning that might improve patient safety, their experiences or outcomes in the future. Organisations are free to decide on the most appropriate way to undertake an investigation.  The framework helps organisations determine the level of investigation that would be appropriate based upon the patient outcome i.e. no harm – death, with higher impact outcomes receiving more detailed investigations.  The Regulations set out the minimum standards for NHS Wales PSI investigations.
