What is a patient safety incident (PSI)?

A PSI is an unintended or unexpected incident, which either led to harm or could have harmed one or more patients.

Harm does not need to have occurred in order for an incident to be reported.  PSI’s are generally incidents reported internally by staff working in NHS organisations.

All cases of nosocomial COVID-19 in NHS Wales are considered to be patient safety incidents.
